Registration and Waiver Form Entry

This document concerns your legal rights and obligations regarding entry to GRAVITATE OSAKA (hereinafter referred to as "the Facility") and participation in activities at the Facility, organized by Gravity Park Holdings Co., Ltd. (hereinafter referred to as "the Organizer"). Please read this document carefully, understand its contents, and sign below before participating in the activities.

(1) I, the undersigned, hereby agree to abide by the rules and regulations established by the organizer (hereinafter referred to as the "Rules and Regulations"), as well as the laws and regulations of this facility, this document, and others. I hereby pledge to obey all laws and regulations, as well as the instructions of the organizer, its directors, employees, management staff, and all other parties involved in the operation of the facility (hereinafter referred to as "the organizer, etc."). I pledge to follow the instructions of the organizer, its officers, employees, management staff, and all other persons involved in the operation of the facility (hereinafter referred to as "the organizer, etc.").

(2) I understand that there is a risk of injury and a risk of life-threatening injury due to the nature of these activities if I am not careful in attempting various activities in the facility. I am aware that these risks are inherent in such activities and I accept that these risks may arise, even if they are specifically unforeseeable risks.

(3) I confirm that I am in good health to safely participate in the activities at this facility and that I am not under the influence of alcohol or the use of illegal or prescription drugs.

(4) In the event that I am injured in the facility, I agree in advance that the organizers or a physician may administer first aid to me, and I agree to abide by the decision of the organizers as to whether or not I may continue to participate in any of the activities at the facility.

(5) I agree to indemnify and hold harmless the organizer, etc., in the event that I am injured while participating in an activity at the facility and later have to undergo medical treatment, if the injury was caused by the risk inherent in participation in the activity as stipulated in Section 2, by the participant's violation of the rules and regulations, laws, or instructions of the organizer, etc., or if the injury was caused by a reason not attributable to the intentional act or negligence of the organizer, etc. I agree to indemnify and hold harmless the organizer, etc. from and against any and all liability arising out of my participation in the activity, if the injury is caused by the participant's violation of the rules, laws, regulations, or instructions of the organizer, etc., or if the injury is caused by reasons not attributable to the intention or negligence of the organizer, etc. I also agree to indemnify the organizer, etc., the facility, or any third party for any damage caused by any cause attributable to me.

(6) I confirm that I will take full responsibility for any personal belongings, equipment, etc. that I bring into the facility, and that the organizers will not be held liable for any loss, damage, or theft of such belongings, equipment, etc.

(7) I agree that the organizers may, at their sole discretion, cancel any activity at the facility in the event that they determine, due to force majeure reasons such as natural disasters, inclement weather, or poor facilities, that the activity cannot be continued or operated, or that the safety of the participants or spectators cannot be ensured.

(8) I agree that if I cancel an activity for my own reason after I have been fitted with safety equipment and have entered the facility, the participation fee will not be refunded. In the event that the organizer decides to cancel an activity after the participant has entered the facility, the participation fee will be refunded.

(9) I agree that my likeness (photo and video) taken while participating in any of the activities at the facility may be used for the purpose of publicity, advertising, and promotional activities in the facility's brochures, newspapers, magazines, television, internet, etc., worldwide, for any period of time without compensation.

(10) I agree that if any dispute arises regarding my rights and obligations under this Agreement, the district court with jurisdiction over the location of the Sponsor's head office shall be the exclusive court of first instance.

※ If an applicant is under 18 years of age at the time of application, the consent of a legal representative such as a parent or guardian is required. (Those who have not yet reached 13 years of age are not allowed to participate.)

※ Personal information of participants obtained by the organizer will be used only for the purpose of appropriate operation of the facility, provision and improvement of services, and will be properly managed to prevent leakage, falsification, or unauthorized use.

※ Be sure to enter the information in person.

(11) I will not intentionally touch or remove any safety equipment (harnesses, carabiners, buckles, etc.) that I am wearing without staff instruction because of the serious risk to my life.

(12) I understand that if I enter our facility after being fitted with safety equipment, and for my own reasons, I stop the activity midway through, I will not be entitled to a refund of the entry fee for any reason whatsoever.

(13) I understand that if the organizers determine that it is difficult to conduct or continue the activity due to mental or physical reasons, such as crying, freezing up, etc., the organizers may stop the activity at their discretion, regardless of my own intentions or the wishes of my guardian. I agree that 100% of the participation fee will be charged even in such cases.

(14) I agree that the organizers shall not be held liable for any damages or issues arising from any false statement regarding my age in this document.

• I have read and understood the "GRAVITATE OSAKA Terms and Conditions" and the above items, and I agree to register and participate in the activities.
